We would like to thank you for giving us the opportunity to care for your furry family member! Our care and dedication starts from the day you bring your adorable new pet (puppy, kitten or rescue) through our doors, and extends to that hardest of days when you trust us to help and guide you through the inevitable goodbye.
For ease of scheduling your first appointment, please call our clinic rather than requesting one online to ensure we get all pertinent information to establish your account.
Once your appointment is scheduled, we ask that you fill out our New Patient Registration Form and either email it to us at email@example.com or bring it with you to your appointment. This form can be found under our Online Forms tab. We ask that you also bring records for your pet from your previous veterinarian. If you do not have records from your previous vet that is no problem. We can also call a previous clinic to get records for you if you prefer.

- Care Credit
Please note that payment is due at the time of service and we do not offer any form of payment plans except Care Credit. For more information about Care Credit please see our Payment Options tab or ask one of our receptionists at your appointment.
We ask that prior to entering the facility, for the safety of all patients and clients, your pet be on a leash or in a carrier. If you do not have a leash, please ask our receptionists to provide you with one.
